Try a sample MCQNeonatal surgery

A term newborn has bilious vomiting on day 1. The abdominal X-ray shows a "double bubble" with no distal bowel gas. What is the most likely diagnosis?

Duodenal atresia. A double bubble (dilated stomach and proximal duodenum) with no gas beyond it suggests complete duodenal obstruction. Look for trisomy 21, which is present in about a third of cases. Malrotation can look similar, but usually some distal gas is seen and it needs urgent surgery.
